CHANGES TO NEW PATIENT REGISTRATION PROCESS

WITHIN THE Three Towns, West Kilbride and Kilwinning

 

There is a well recognised GP Workforce challenge both Nationally and here in Ayrshire. We in Ayrshire are trying to alleviate the effects on patients and have had to make some changes to how patients register in the above areas.

 

By completing a GPR (General Practice Registration for New Patients) patients are requesting to be added to our Practice List. Due to our list being closed, we are unable to guarantee that you will be added to our own list. This GPR will be sent to the Health Board who will allocate patients to a Practice within this area.

 

If the GPR application is approved to be added to our list here at Kilwinning Medical Practice we will contact patients within a couple of days.

 

However, patients may be allocated to another Practice within this area who will contact patients direct. Patients will have to remain with this allocated GP Practice to ensure patients’ care is well co-ordinated.

New Patient Registration.

docandcomputerThe doctors welcome new patients who live within our practice area KA13 postal Codes.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire(see below for over 15yrs & under 15yrs questionnaires) and are offered a consultation with the doctor. 

Medical treatment is available from the date of registration.  Please contact reception for further information.

YOU WILL NEED :   Registration Form & questionnaire completed - HAND QUESTIONNAIRES INTO THE SURGERY WITH THE SIGNED REGISTRATION FORM BEFORE WE CAN COMPLETE YOUR ACCEPTANCE ON TO OUR PRACTICE LIST OF PATIENTS.
